La neuralgia occipital es una enfermedad frecuente que se debe casi siempre a afectaci n del nervio occipital mayor o nervio de Arnold. Puede manejarse con f rmacos por v a oral, o con infiltraci n anest sica del nervio, la cual est infrautilizada. Se presentan 2 casos de pacientes que no respondieron al primer tratamiento y lo hicieron de forma satisfactoria a la infiltraci n. Se analizan los principales conceptos sobre anatom a, fisiopatolog a, etiolog a y cl nica de la enfermedad, as como de la t cnica de la infiltraci n anest sica. Occipital neuralgia is a common disease most oftenly caused by greater occipital nerve or Arnold nerve affection. It can be managed with oral medications or anesthetic infiltration of the nerve, which is underused. We present 2 cases of patients who did not respond to initial treatment and did so successfully to infiltration.
